Question:  I would like to request a copy of this RFP.  I was not able to find it online.  Could you please forward me an electronic copy?

Answer:     The RFP is available online at www.labavn.org or www.lacity.org/lahd, or at the Los Angeles Housing Department, 1200 W. 7th St., 1st Floor, Los Angeles, CA 90017.

Question:  Is a certificate of good standing required if an organization is not non-profit?

Answer: Please refer to Page 18 of the Request for Proposals document. Item 2 - Secretary of State Documentation - states: All contractors are required to submit one copy of their Articles of Incorporation, partnership, or other business organizational documents (as appropriate) filed with the Secretary of the State. Organizations must be in good standing and authorized to do business in California.

Question: Our company was recently incorporated and we just sent our statement of information to the Secretary of State, but have not yet received a "filed" stamped copy. Are we still eligible to submit a proposal?

Answer: You may still submit a proposal. However, if your organization is selected as a contractor, you must provide a finalized copy of its Articles of Incorporation at the time of entering into a contractual agreement with the City of Los Angeles, showing that your organization is in good standing and authorized to do business in the State of California.

Question: The attachment/exhibits section of the proposal checklist has a section entitled "Work Sample." Can you provide examples or insight into the type of documentation needed to fulfill this requirement?

Answer: A work sample may take various forms, including a project design, plan, or studies, etc. that you have submitted previously for another assignment or contract. It may also include a report or research assignment that your organization has produced. Essentially, the work sample may be any written and/or graphical material that shows the capabilities of your organization as they apply to providing REAP and UMP outreach services. Please be advised that the information you submit will be subject to the California Public Records Act. Therefore, you must take into consideration whether this information is subject to any proprietary rights. Please see Page 14 of the RFP, Item 9 - Proprietary Interests of the City - for further details.

Question: Do the attachments/exhibits (list of personnel/organization chart; job description; professional reference work samples) count toward page limits in any of the narratives or are they separate?

Answer: These are part of the narrative, but you may submit additional information in the optional Narrative 4.
